Everwell Family Clinic welcomes individuals and families ages three and older, including local residents and visitors to the area.

New patients may be asked to complete registration documents, provide basic medical history information, and review clinic policies before receiving care.

Completing forms ahead of time can reduce delays and help the clinic focus more of your visit on your questions and healthcare needs.

What to bring to your visit

Bringing accurate information helps the clinic understand your needs, avoid unnecessary delays, and provide safer care.

Photo identification Bring a valid photo ID for registration and identity verification.
Insurance card Patients using insurance should bring their current card and plan information.
Medication list Include prescriptions, over-the-counter medications, vitamins, and supplements.
Allergy information Include known medication allergies and previous serious reactions.

Payment information

Understand your options before your visit

Everwell is intended to serve insured, uninsured, cash-pay, and membership patients. Final insurance participation and pricing information will be published once confirmed.

Patients should verify coverage directly with their insurance company. Insurance participation does not guarantee that every service will be covered.

Insurance Patients Bring a current insurance card and verify plan benefits before receiving care.
Cash-Pay Patients Ask the clinic about current self-pay pricing and available payment methods.
Membership Patients Review which services are included and which may require additional payment.

Transferring and requesting health information

Medical records should be transferred only through clinic-approved, secure methods.

Send Records to Everwell

Contact the clinic for secure instructions before sending outside medical records, imaging, laboratory results, or other protected health information.

Request Everwell Records

Patients may be required to complete a signed authorization before records can be released to another provider or individual.

Do Not Use General Email

Do not send private medical documents, Social Security numbers, or insurance-card images through an unsecured contact form or general email address.

Minor Patients
